Quality Manager, Home Health – (Full-time)

This position is responsible for overseeing and coordinating the Home Health quality assurance and performance improvement (QAPI) program and other assigned duties.  Current RN licensure in PA and minimum one year of general nursing experience , preferably in Home Health.  Excellent organizational, time management, and teamwork skills a must.  Must also be very detail oriented.

Home Health RN – (Full-time)

The RN provides professional skilled nursing care to home health patients in their home. Current RN licensure in PA and general nursing experience in home health, medical, surgical, or critical care required.  BSN preferred.  Excellent organizational, time management, and teamwork skills a must.

 

Hospice RN- (Full-time)

The RN provides professional skilled nursing care to hospice patients in their home. Current RN licensure in PA and general nursing experience in hospice, oncology, home health, medical, surgical, or critical care required.  BSN preferred.  Excellent organizational, time management, and teamwork skills a must.

 

Aide, Home Care Plus – (Flexible hours & weekends)

The Home Care Plus Aide provides personal care and light housekeeping services to patients in their homes. High school diploma or equivalent. CNA or proof of NA training required. Minimum 1 year of work experience. Experience in home care preferred.

 

Home Health/Hospice Aide (HHA) – (Full-Time)

Provides personal care services to patients in their home, nursing facility, inpatient unit, etc. and performs care plan tasks under the supervision of an RN.   High school diploma or equivalent. CNA or proof of NA training required. Minimum 1 year of work experience. Experience in home care preferred.

 

Spiritual Counselor – (PRN, Flexible Hours)

The Spiritual Counselor provides spiritual counseling services to Hospice patients, families and caregivers. Must be ordained, consecrated, commissioned and/or licensed and in current good standing with appropriate religious body. A degree in divinity, theology, spiritual care, ministerial studies, psychology, sociology or social work; a minimum of 1 to 2 C.P.E.’s from an accredited program; and experience in grief or loss counseling required. A minimum of two years experience in home health care preferred.
